A couple of years ago, I bought a paper copy -- just for the heck of
it, to rack up next to my copies of R{0,4,5}RS -- at a local technical
bookstore. IIRC, it cost me over $60(!!). The front cover says "IEEE
Std 1178-1990", and inside, "Approved December 10, 1990".

But I seem to have also manually penned in an addendum that says
"Reaffirmed December 12, 1995". IIRC, that was on a blow-in errata
sheet that came with it. (I guess I penned it in so I wouldn't lose the
info in case I misplaced that little piece of paper... which is indeed
now missing!) The IEEE Web site lists the name as "1178-1990(R1995)",
so that's probably still the status. Hmmm... Here's some more:

p.s. Besides getting it from a technical bookstore, a footnote on page 8 says:

    "[2] IEEE publications are available from the Institute of
     Electrical and Electronics Engineers, Service Center,
     445 Hoes Lane, P.O.Box 1331, Piscataway, NJ 08833-1331, USA."
